SurveySparrow. You can set the day and time, and share to your email list on a weekly, monthly or yearly basis. You’ll only need to configure the survey initially. Hope this helps :).

SurveySparrow: The UI feels like it was built with TypeForm and they claim to have a more “conversational” style to surveys. The feedback chatbot seems really interesting even though I have yet to actually demo it. Pricier than SurveyMonkey but feels like it has more robust data collection capabilities for savvy users.
